What this does, and what it does not

A person reads the report before it goes any further. The form is open to anyone, so nothing on it is taken on trust.

Once it stands up, the IMEI is held on file with the date and the crime reference you gave us.

It does not bar the handset. Only the networks can stop an IMEI connecting, which is why the call to yours matters far more than this form.

Your network comes first

Ask your network to bar the IMEI. Networks share those reports between them, so in most countries the handset is then refused calls and mobile data wherever it surfaces. That is the step that acts on the phone itself, and nothing on this page can do it for you.
